What Is the Diabetic Shoe Program?

The Medicare Therapeutic Shoe Bill allows eligible patients with diabetes to receive specialized footwear and custom orthotic inserts designed to protect their feet and reduce the risk of serious complications. These therapeutic shoes are built with extra depth, soft protective interiors, and supportive insoles that help prevent pressure spots, blisters, and ulcers.


A rocker-bottom sole and rigid shank help realign ground forces and encourage a healthier, more natural gait. When paired with heat-moldable inserts, these shoes offer enhanced protection for sensitive feet and improve overall comfort and mobility.


Medicare typically covers one pair of extra-depth shoes and three pairs of inserts per year, covering 80% of eligible costs. Secondary insurance may help cover the remainder. Many private insurance plans also provide coverage for patients who meet the criteria.

Who Is Eligible?

Therapeutic shoes, inserts, and shoe modifications are covered if the patient:

  • Has diabetes mellitus, and
  • Has one or more of the following conditions:
  • History of partial or complete foot amputation
  • Previous foot ulceration
  • Pre-ulcerative callus
  • Peripheral neuropathy with callus
  • Foot deformity
  • Poor circulation


The program’s purpose is to reduce the risk of diabetic foot complications and help patients remain active, independent, and safe.

  • Get regular podiatric checkups
  • Measure feet properly when buying shoes
  • Avoid extreme temperatures
  • Check shoes for rough seams or objects

Don’t:

  • Go barefoot
  • Wear high heels, sandals, or pointed shoes
  • Drink alcohol excessively
  • Wear tight socks or legwear
  • Try to remove corns or calluses yourself
  • Smoke
  • Soak feet in hot water
